NEW PHARMACY - obtain a new pharmacy license for a facility located in Texas

EXISTING PHARMACY LICENSE - information for changes to existing licenses (e.g. Change of Ownership, Change of Managing Officers, Change in Name/Location, Change of Pharmacist-in-Charge (PIC), and Proper Closing of a Pharmacy).

TEXAS DEPARTMENT OF STATE HEALTH SERVICES - Drugs and Medical Devices.  information regarding Texas Wholesale/Distributor Licensing.

TEXAS DEPARTMENT OF PUBLIC SAFETY (DPS) - obtain Controlled Substance registration.

DRUG ENFORCEMENT ADMINISTRATION (DEA) - Obtain controlled substance registration, as well as forms (e.g. Affidavit for a Transfer of a Pharmacy, Power of Attorney for DEA Order Forms, Affidavit Form, Form 41 - Destruction of Controlled Substance, DEA Form 106 - Theft or Loss of Controlled Substances, etc.).

REQUIREMENTS FOR OTC SALE OF EPHEDRINE AND/OR PSEUDOEPHEDRINE
Texas Law requires all pharmacies that sell over-the-counter medications containing ephedrine and/or pseudoephedrine (PSE) to participate in a statewide, real-time electronic PSE monitoring program. Click here to obtain information on how to report to the PSE monitoring program.
