Change of Address and Employment



Licensed pharmacists, registered technicians, and interns may submit changes of address, telephone number, or employment online.

PIC Changes CAN NOT be made online.

Note: Employers NOT licensed with the Texas State Board of Pharmacy CAN NOT be added to employment information and are not required to be reported to the board.

A new system implemented in June 2011 allows users to “register for a secure online account” to access license/registration information.

Click on the following link to register for a secure online account or simply log-in if you created an account as of June 2011:


ONLINE Change of Address, Telephone Number and Employment

 

For Employment Changes:

Enter in user name and password (If you do NOT have one register as a new user, if you DO have one but have forgotten the password click “forgot password ”link , return to this site with password and continue with following instructions after changing your password)

If you do not see your license/registration number, select the function to add your license/registration number and follow the instructions. If /when your license/registration number is shown on the page click the link and utilize the directions below:

The functions on the left will change and one of the selections will be ‘Employment relationships’ Click and follow the directions—if you are adding a pharmacy you will need your employer’s pharmacy license number to add your employment. When you have finished entering all of your employer’s information select ‘next’. You will be on the employment summary screen where you will be allowed to ‘submit’ your change of employment.

Changes will show on the License and Verification page the following day.

For Address Changes:

Enter in user name and password (If you do NOT have one register as a new user, if you DO have one but have forgotten the password click “forgot password ”link , return to this site with password and continue with following instructions after changing your password)

If you do not see your license/registration number, select the function to add your license/registration number and follow the instructions. If /when your license/registration number is shown on the page click the link and utilize the directions below:

The functions on the left will change and one of the selections will be ‘Change of Address’ Click and follow the directions— You must enter 2 addresses! A mailing address and a public address. The addresses may be the same but you must have 2 addresses on file. Follow the directions and ‘submit’ your changes.

Changes will show on the License and Verification page the following day.

Pharmacies May Not Delete Employment Information Online - Send an email request to:mailto:mailing@tsbp.state.tx.us
